Background:

  • Organ transplant waiting lists exceed available donor organs.
  • Non-standard risk donors, including those with small renal tumors, are increasingly considered.
  • Bench tumor excision allows utilization of kidneys from donors with incidental renal masses.

Purpose of the Study:

  • To analyze the outcomes of kidney transplantation using grafts from donors with small incidental renal tumors.
  • To evaluate the functional and oncologic results of such transplantations.

Main Methods:

  • Retrospective analysis of a prospective database.
  • Inclusion of donors with incidental renal masses undergoing bench tumor excision.
  • Data collected between January 2007 and August 2018.

Main Results:

  • Twenty kidney transplantations were performed (13 affected, 7 contralateral kidneys).
  • Mean tumor diameter was 12.7 mm; 15/17 were renal cell carcinoma, all with negative margins.
  • No delayed graft function; 69-month follow-up showed no tumor recurrence in donors or recipients.

Conclusions:

  • Kidneys with small incidental tumors are a viable option for selected kidney transplant recipients.
  • Bench surgery excision followed by transplantation yields good functional and oncologic results.
  • Longer follow-up studies are necessary to validate these findings.
