    Summary

    Diffuse fasciitis with eosinophilia developed in a patient post-bone marrow transplant for acute lymphoblastic leukemia. Treating chronic graft-versus-host disease improved these fasciitis symptoms.

    Background:

    • Diffuse fasciitis with eosinophilia (DFE) is a rare condition.
    • DFE has been associated with hematologic disorders.
    • Allogeneic bone marrow transplantation (BMT) is a treatment for hematologic malignancies.

    Observation:

    • A patient developed DFE months after BMT for acute lymphoblastic leukemia (ALL).
    • The patient also had chronic graft-versus-host disease (cGVHD) following BMT.
    • Clinical, laboratory, and histologic findings were consistent with DFE.

    Findings:

    • DFE can occur in the context of cGVHD after BMT.
    • Improvement in cGVHD symptoms correlated with improvement in DFE symptoms.
    • This suggests a potential link between cGVHD and DFE pathogenesis.

    Implications:

    • This case highlights a potential complication of allogeneic BMT.
    • Understanding the link between cGVHD and DFE may inform treatment strategies.
    • Further research is needed to elucidate the mechanisms underlying DFE in transplant recipients.
