Multiple sclerosis (MS) patients exhibit altered attention network connectivity during tasks, differing from healthy controls. This altered connectivity correlates with cognitive performance deficits in MS.

Background:

  • Multiple sclerosis (MS) can impair cognitive functions, particularly attention.
  • Existing research on attention network deficits in MS primarily focuses on resting states, with limited data during task performance.

Purpose of the Study:

  • To examine functional connectivity within and between attention networks during a visual attention task in relapsing-remitting multiple sclerosis (RRMS) patients.
  • To correlate network connectivity with cognitive performance in RRMS patients.

Main Methods:

  • Task-based functional magnetic resonance imaging (fMRI) was employed in 23 RRMS patients and 29 healthy controls.
  • Tensor-independent component analysis (TICA) was used to analyze fMRI data and assess functional connectivity.
  • Cognitive function was evaluated using the Brief International Cognitive Assessment for MS (BICAMS) battery.

Main Results:

  • Healthy controls showed higher scores in visual/attention and executive components compared to RRMS patients.
  • RRMS patients exhibited increased connectivity between visual/attention and default-mode networks, linked to poorer visuospatial memory.
  • Reduced connectivity was observed between the right intraparietal sulcus (rIPS) and frontal eye field (rFEF) in RRMS patients, correlating with lower scores on the Symbol Digit Modalities Test (SDMT) and BVMT-R.

Conclusions:

  • Attention networks demonstrate altered functional connectivity during task engagement in RRMS patients.
  • The degree of altered connectivity scales with the severity of cognitive disability in multiple sclerosis.
