Collaborative study for the establishment of Etanercept Biological Reference Preparation Batch 1

Background:

• Etanercept, a TNF receptor II-Fc fusion protein, requires standardized potency assays.
• Establishing international and European reference standards is crucial for consistent biological drug evaluation.

Purpose of the Study:

• To evaluate candidate preparations for their suitability as a World Health Organization (WHO) International Standard (IS) and European Pharmacopoeia (Ph. Eur.) Biological Reference Preparation (BRP) for Etanercept.
• To assign an official potency to the established reference standard.

Main Methods:

• Three Etanercept preparations were formulated and lyophilized.
• Seven laboratories performed collaborative testing using an in vitro cell-based bioassay (TNF-α neutralisation) as per the Ph. Eur. monograph.
• Results were compared with other cell-based assays used in parallel studies.

Main Results:

• Candidate preparation 13/204 was successfully established as the first WHO IS for Etanercept.
• Preparation 13/204 was also deemed suitable and adopted as the Ph. Eur. BRP batch 1.
• An assigned potency of 10,000 IU per ampoule for TNF neutralisation activity was determined for preparation 13/204.

Conclusions:
