    Liver transplantation is a viable treatment for severe liver trauma from accidents. This procedure offers a chance for survival in patients with irreparable liver damage.

    Background:

    • Blunt trauma from motor vehicle accidents can cause massive hepatic injuries.
    • Nonreconstructable injuries to the porta hepatis and destruction of the right liver lobe present significant surgical challenges.

    Observation:

    • Two patients with severe blunt hepatic trauma underwent emergency surgery.
    • Hemorrhage was controlled, but both patients had nonfunctional liver remnants.

    Findings:

    • Both patients successfully received liver transplants.
    • One patient survived 16 months post-transplant, while the other succumbed to cytomegalovirus infection 7 weeks later.

    Implications:

    • Liver transplantation is a feasible option for patients with lethal hepatic injuries.
