Background:

The recently introduced two-vector system for periodontal disease classification presents challenges in accurately assigning, staging, and grading periodontitis.
Fundamental debates exist regarding the precise application of this new classification system.

Observation:

Two clinical cases of periodontitis (Stage III Generalized Grade B and Stage IV Generalized Grade B) with similar clinical and radiographic findings were analyzed.
The diagnostic thought process, incorporating clinical judgment, was detailed for each case.

Findings:

The application of the new periodontitis classification system necessitates careful clinical judgment, especially in ambiguous cases.
The presented cases highlight the importance of integrating clinical expertise with the classification guidelines for accurate diagnosis.

Implications:

Clinicians can utilize these case examples to enhance their understanding and application of the new periodontitis classification system.
This approach aims to clarify implementation questions and improve diagnostic accuracy in periodontitis management.
