Background:

Parathyroid cysts are uncommon cervical or mediastinal lesions, predominantly affecting women in their fourth and fifth decades.
These non-functioning cysts are typically asymptomatic and often discovered incidentally during imaging or surgery.
Distinguishing parathyroid cysts from thyroid pathology poses a significant diagnostic challenge.

Observation:

The case presents a parathyroid cyst that clinically mimicked a thyroid nodule.
The patient's presentation involved a palpable neck mass.
Initial diagnostic workup did not definitively differentiate the cyst from thyroid tissue.

Findings:

Intraoperative findings were crucial for suspecting the parathyroid origin.
Histopathological examination confirmed the diagnosis of a parathyroid cyst.
The cyst was found to be non-functioning, consistent with typical parathyroid cyst characteristics.

Implications:

Accurate preoperative diagnosis of parathyroid cysts remains difficult, often requiring intraoperative assessment.
Increased awareness of parathyroid cysts can aid clinicians in differential diagnosis of neck masses.
Histopathological confirmation is essential for definitive diagnosis and appropriate management of parathyroid cysts.
