    Summary

    Two patients with thalamic infarcts showed no memory loss, suggesting that dorsomedial nucleus (DM) damage alone may not cause amnesia. Intact mamillothalamic tracts might preserve memory function.

    Background:

    • Medial thalamic lesions are often associated with significant memory impairments (amnesia).
    • The dorsomedial nucleus (DM) and mamillothalamic tract are critical for memory function.

    Observation:

    • Two patients with nonhemorrhagic thalamic infarcts underwent comprehensive cognitive assessment.
    • Neither patient exhibited deficits in verbal or nonverbal memory recall.
    • MRI revealed unilateral DM lesion in one patient and bilateral DM lesions in the other, with intact mamillothalamic tracts.

    Findings:

    • Despite thalamic lesions, particularly involving the dorsomedial nucleus (DM), patients did not develop amnesia.
    • The extent of DM damage (15% unilateral, 15% bilateral) may have been insufficient to cause amnesia.
    • Preservation of the mamillothalamic tract might have prevented amnesia.

    Implications:

    • These findings challenge the direct causal link between isolated DM lesions and amnesia.
    • Amnesia may require damage to additional structures, such as the mamillothalamic tract or anterior nucleus, in conjunction with DM lesions.
    • Further research is needed to elucidate the precise neural circuitry underlying memory and the conditions under which thalamic damage leads to amnesia.
