Area of Science:

  • Urology
  • Neurosurgery
  • Public Health

Background:

  • Delayed ejaculation (DE) is a condition causing distress due to delayed or absent ejaculation.
  • No prior studies have documented DE following lumbar spine surgery.
  • This study investigates the incidence of DE after lumbar spine surgery.

Purpose of the Study:

  • To determine the national incidence of delayed ejaculation after lumbar spine surgery.
  • To identify risk factors associated with developing DE post-lumbar spine surgery.

Main Methods:

  • A retrospective cohort study using the Optum De-identified Clinformatics Database (2003-2017).
  • Included adult men undergoing lumbar spine surgery (e.g., ALIF, PLF).
  • DE diagnosis within 2 years of surgery was the primary outcome; multivariable logistic regression identified associated factors.

Main Results:

  • Over 117,918 men underwent 162,646 lumbar spine surgeries.
  • The overall incidence of DE was 0.09%, highest in ALIF (0.13%).
  • Tobacco smoking (OR=1.47) and obesity (OR=1.56) were associated with increased DE risk.

Conclusions:

  • Delayed ejaculation is a rare but distressing complication of thoracolumbar spine surgery.
  • Patients should be monitored for DE symptoms postoperatively.
  • Further research may explore preventative strategies for DE in this population.
