Summary
11C-metomidate PET (11C-MTO) is not a reliable noninvasive alternative to adrenal venous sampling (AVS) for classifying primary aldosteronism (PA). The study found 11C-MTO-PET to be clinically suboptimal and inferior to AVS for PA subtype classification.

- 11C-MTO-PET demonstrated clinically suboptimal concordance with AVS for PA subtype classification.
- The imaging modality did not outperform adrenal CT and was found to be inferior to AVS.
- In a subgroup with CYP11B2-positive adenoma, 11C-MTO-PET identified only 53% of cases.
