Summary

A giant intrathoracic mass, initially suspected as lipoma, was surgically removed from a 50-year-old female. This largest-ever documented case was confirmed as a 6kg liposarcoma, highlighting rare mediastinal tumors.

Observation:

  • A 50-year-old female presented with severe dyspnea and dysphagia.
  • Chest computed tomography (CT) revealed a giant bilateral posterior mediastinal mass compressing mediastinal structures and the heart.
  • Initial CT-guided biopsy suggested a lipoma.

Findings:

  • Surgical removal via left posterolateral thoracotomy successfully excised the entire mass, weighing 6 kg and measuring 42 × 25 × 10 cm.
  • Histologic examination confirmed the mass as a grade 1 liposarcoma.
