Summary

Quantitative electroencephalography (qEEG) can detect early cerebral ischemia. Relative-alpha-variability (RAV) changes on qEEG may precede Alpha-Delta-Ratio (ADR) declines, offering a novel marker for ischemia detection.

Background:

  • Quantitative electroencephalography (qEEG) is underutilized for early cerebral ischemia detection.
  • Cerebral ischemia is a critical complication of aneurysmal subarachnoid hemorrhage (aSAH).

Observation:

  • A patient with aSAH and vasospasm underwent continuous electroencephalogram (EEG) monitoring.
  • EEG monitoring revealed early signs of cerebral ischemia.

Findings:

  • A decline in the Alpha-Delta-Ratio (ADR) indicated cerebral ischemia.
  • Relative-Alpha-Variability (RAV) changes were observed to precede ADR changes.
  • This suggests RAV may be a more sensitive early indicator of ischemia.

Implications:

  • RAV's early detection capability could improve clinical management of cerebral ischemia.
  • Findings may extend to monitoring for early reocclusion or reperfusion injuries post-thrombectomy.
  • Highlights the potential of qEEG in critical care neuro-monitoring.
