Alveolar hydatid disease, caused by Echinococcus multilocularis, can spread to distant sites. This case report details the first documented instance of ocular involvement, specifically a choroidal mass, in a patient with this parasitic infection.

    Area of Science:

    • Parasitology
    • Ophthalmology
    • Infectious Diseases

    Background:

    • Alveolar hydatid disease (AHD), caused by Echinococcus multilocularis, is a severe parasitic infection.
    • Hepatic infestation is primary, but local and hematogenous spread to distant organs can occur.
    • Complications include hepatic failure, local invasion, and metastasis to the brain or lungs.

    Observation:

    • A 67-year-old Alaskan Eskimo with a history of AHD presented with decreased visual acuity.
    • Ocular examination revealed a choroidal mass in the right eye.
    • Pathologic examination confirmed parasitic membranes consistent with Echinococcus multilocularis.

    Findings:

    • This case represents the first documented instance of ocular involvement in alveolar hydatid disease.

    Related Experiment Videos

  • The patient developed a choroidal mass eight years after initial AHD diagnosis and four years prior to symptomatic cerebral metastasis.
  • Parasitic membranes were identified in the posterior pole of the right eye.
  • Implications:

    • Ocular involvement should be considered in patients with a history of Echinococcus multilocularis infection.
    • Early diagnosis and management of ocular manifestations may improve patient outcomes.
    • This finding expands the known spectrum of E. multilocularis disease presentation.
