Summary

Accidental subdural placement of spinal cord stimulator leads can be successful. Modified parameters provided long-term pain relief in a patient with failed back surgery syndrome, challenging removal necessity.

Background:

  • Spinal cord stimulator (SCS) implantation is a common treatment for chronic pain.
  • Accidental subdural placement of SCS leads is considered a complication requiring lead removal due to presumed poor efficacy.

Observation:

  • A 59-year-old male with failed back surgery syndrome experienced lead migration into the subdural space during an SCS revision procedure.
  • The patient had previously achieved pain control with SCS.

Findings:

  • Instead of immediate removal, modified stimulation parameters were applied to the subdurally placed lead.
  • This approach resulted in excellent and sustained pain relief for the patient.
