A renin-secreting tumor.

Summary

A renin-secreting renal hemangiopericytoma caused resistant hypertension and hypokalemia. Surgical removal normalized blood pressure and electrolyte levels, indicating the tumor was the source of excess renin.

Background:

Resistant hypertension, hypokalemia, and hyperaldosteronism in a young male.
Elevated plasma renin activity (PRA) suggested a renin-producing tumor.

Observation:

A 1.0 cm avascular solid mass was identified in the right kidney via imaging.
Renal vein catheterization confirmed lateralized renin secretion from the right kidney.

Findings:

The resected mass was diagnosed as a renal hemangiopericytoma.
Post-surgery, blood pressure, potassium levels, and PRA normalized.
Both active and inactive renin levels decreased in parallel after tumor removal.

Implications:

Renal hemangiopericytoma can cause secondary hypertension via renin hypersecretion.
Surgical tumor excision is an effective treatment for this condition.
Tumoral renin secretion may be influenced by sympathetic stimuli but resistant to angiotensin II feedback.
