Summary

Spontaneous knee haemarthrosis, often seen in elderly patients with osteoarthritis, may stem from bleeding peripheral arteries. This case highlights these arteries near the lateral meniscus posterior horn as a likely source in active individuals.

Background:

  • Spontaneous knee haemarthrosis is rare, primarily affecting elderly individuals with osteoarthritis.
  • Peripheral arteries supplying the lateral meniscus posterior horn are increasingly implicated as the bleeding source.

Observation:

  • A 62-year-old active male presented with acute left knee pain, swelling, and limited mobility.
  • He had a history of osteoarthritis and prior arthroscopic partial lateral meniscectomy.
  • Clinical presentation included a large haemarthrosis confirmed by aspiration.

Findings:

  • MRI revealed extensive chondral loss, complex lateral meniscus tearing, and loose bodies.
  • Arthroscopy identified a bleeding point, treated with debridement and diathermy.
  • The findings support the hypothesis of bleeding peripheral arteries supplying the lateral meniscus posterior horn.

Implications:

  • This case reinforces the understanding of spontaneous knee haemarthrosis etiology in active patients.
  • It suggests a potential source of bleeding in cases of knee haemarthrosis without apparent trauma.
  • Further investigation into meniscal vascularity may be warranted for managing such conditions.
