Area of Science:

  • Orthopedic Surgery
  • Knee Arthroscopy
  • Meniscal Repair

Background:

  • Meniscal radial root tears compromise knee function and stability, particularly in anterior cruciate ligament-deficient knees.
  • Traditional transtibial pullout techniques for paracentral radial tears can result in nonanatomic and high-tension repairs.

Purpose of the Study:

  • To present two all-inside surgical techniques for the anatomic repair of medial and lateral meniscal radial root tears.
  • To provide technical requirements and tips for successful implementation of these novel repair methods.

Main Methods:

  • All-inside repair using a dedicated meniscal repair device.
  • All-inside repair utilizing an antegrade suture passer.
  • Detailed descriptions of portal placement, stitch configuration, and depth settings for each technique.

Main Results:

  • The proposed all-inside techniques facilitate anatomic reduction and fixation of meniscal root tears.
  • Specific guidance is provided to minimize risks, such as vascular injury during lateral meniscus repair.
  • Recommendations include using at least two stitches with specific depth parameters for optimal repair.

Conclusions:

  • These all-inside techniques represent advancements in the surgical management of meniscal radial root tears.
  • They offer a potentially more anatomic and less tensioned repair compared to traditional methods.
  • Successful application requires careful attention to technical details and portal management.
