Summary

Computed tomography (CT)-guided coil localization (CL) is a safe and effective method for locating difficult lung nodules before surgery. This technique ensures high success rates for subsequent video-assisted thoracoscopic surgery (VATS) wedge resections of scapula-blocked lung nodules (SBLNs).

Background:

Purpose of the Study:

  • To evaluate the clinical efficacy, feasibility, and safety of preoperative computed tomography (CT)-guided coil localization (CL) for SBLNs.
  • To assess the success rate of subsequent video-assisted thoracoscopic surgery (VATS) wedge resection following CL.

Main Methods:

  • A retrospective analysis of 123 patients undergoing CT-guided CL and VATS wedge resection from January 2015 to June 2020.
  • Focus on 12 patients with SBLNs who underwent CT-guided CL.
  • Recorded technical success of localization and VATS, and localization-related complications.

  • CT-guided CL achieved a 100% technical success rate in localizing SBLNs.
  • All patients underwent successful VATS-guided wedge resection without conversion to thoracotomy.
  • One patient (8.3%) experienced asymptomatic pneumothorax, which did not affect the VATS procedure.

  • Preoperative CT-guided CL is a safe, feasible, and effective approach for SBLNs.
  • This method facilitates high success rates for VATS-guided wedge resection of SBLNs.
  • The technique simplifies the surgical management of challenging lung nodules.
