Background:

  • Lumbar disc herniation with fragment migration occurs in 35%-72% of cases.
  • Posterior epidural migration of lumbar disc fragments is an uncommon presentation.

Observation:

  • A 72-year-old male presented with sciatica and motor deficit (Extensor Hallucis Longus Grade 3).
  • MRI confirmed a prolapsed intervertebral disc with suspected posterior epidural migration.
  • During microdiscectomy, the migrated disc fragment was not visualized, and nerve roots appeared free.

Findings:

  • Extensive surgical exploration failed to locate the disc fragment.
  • Suction fluid, filtered through a surgical mop, was histopathologically confirmed as intervertebral disc material.
  • This suggests accidental suction of the migrated disc fragment occurred.
