Nailing unstable pertrochanteric fractures: does size matters?

Summary

This study found no significant differences in outcomes or complications when comparing long versus short cephalomedullary nails for unstable 31A2 femoral fractures. However, longer nails were linked to increased blood loss, longer operative times, and more anterior knee pain.

Purpose of the Study:

  • To compare midterm outcomes and complication rates of two Gamma3 nail lengths for unstable 31A2 fractures.
  • To evaluate the efficacy and safety of Long Dynamic Gamma3 Nail (LGN) versus Short Static Gamma3 Nail (SGN).

Main Methods:

  • Retrospective study of 104 patients with 31A2 fractures treated with Gamma3 nails.
  • Comparison of LGN (n=52) and SGN (n=52) groups with a minimum 1-year follow-up.
  • Analysis of preoperative, postoperative, functional, radiological, and complication data.

  • SGN is recommended due to lower blood loss, shorter operative time, and reduced risk of anterior knee pain and reoperation.
  • Further research may explore optimal nail selection based on fracture characteristics.
