Area of Science:

  • Neurology
  • Radiology
  • Medical Imaging

Background:

  • Parkinsonism diagnosis relies on imaging techniques like Dopamine Transporter Single-Photon Emission Computed Tomography (DAT SPECT).
  • Quantitative analysis of DAT SPECT scans aids in differentiating parkinsonism from other conditions.
  • Software tools are essential for processing and interpreting DAT SPECT images.

Purpose of the Study:

  • To compare a commercial DAT SPECT software (Datquant®) with a free software (BasGanV2).
  • To evaluate the correlation of software outputs with a clinical motor score (MDS-UPDRS-III).
  • To assess the concordance of software results with expert visual readings.

Main Methods:

  • Analysis of 151 DAT SPECT scans from outpatients with suspected parkinsonism.
  • Image post-processing using Datquant® and BasGanV2 software.
  • Comparison of software results with expert readings and MDS-UPDRS-III scores in a subset of patients.

Main Results:

  • High correlation (r = .706–.887) observed between Datquant® and BasGanV2 for SBR, P/C ratio, and asymmetries.
  • Correlation with MDS-UPDRS-III was higher for caudate SBR and generally better with BasGanV2.
  • Concordance with expert reading was 84.1% for Datquant® and 80.8% for BasGanV2, improving with P/C ratio inclusion.

Conclusions:

  • Both Datquant® and BasGanV2 are effective for semi-quantification of DAT SPECT scans.
  • The software tools demonstrate similar performance in analyzing DAT SPECT images.
  • Users should be aware of each tool's strengths and limitations for optimal DAT SPECT interpretation.
