- To investigate olfactory performance as a potential biomarker for differentiating ET and TDPD.
- To assess the utility of olfactory bulb volumetry (OBV) alongside olfactory testing.

- 36 ET patients, 22 TDPD patients, and 24 healthy controls (HCS) participated.
- Participants underwent the Sniffin' Sticks Test (SST) for olfactory function and olfactory bulb volumetry (OBV).

- TDPD patients showed significant deficits in SST threshold, discrimination, and identification compared to ET and HCS.
- ET patients also exhibited significant olfactory deficits compared to HCS.
- TDPD patients had significantly reduced OBV compared to ET and HCS.
- A TDI score of 25 demonstrated 94% sensitivity and 91% specificity in differentiating TDPD from ET.
Conclusions:
- Olfactory assessment provides a rapid, safe, and easily applicable method for differentiating TDPD from ET.
- Olfactory testing can serve as a valuable biomarker in cases where ET and TDPD diagnosis is uncertain.
