Summary
Tuberculosis (TB) of the posterior pharyngeal wall is rare and can mimic cancer. Early diagnosis and anti-tubercular therapy are crucial for successful treatment of this uncommon throat condition.

- Tuberculosis (TB) affecting the posterior pharyngeal wall (PPW) is exceptionally rare.
- It can present as a diagnostic challenge, often mimicking malignant growths, particularly in older individuals.
Observation:
- A case of a 36-year-old female with a persistent, nonhealing throat ulcer is presented.
- Clinical examination revealed an ulceroproliferative lesion on the PPW; imaging showed localized uptake, and biopsy confirmed epithelioid granulomas and bacilli consistent with TB.
Findings:
- Microscopic examination of the PPW lesion revealed characteristic granulomatous inflammation with Langhans giant cells.
- Auramine-rhodamine staining identified bacilli, confirming tuberculosis as the etiology of the pharyngeal lesion.
Implications:
- Posterior pharyngeal wall tuberculosis is an uncommon diagnosis that requires consideration in differential diagnoses.
- Prompt identification and treatment with anti-tubercular therapy can lead to complete cure, avoiding unnecessary surgical or oncological interventions.
