Vertebral artery dissection (VAD) following a sneeze led to cerebellar ischemia in a 37-year-old man. Treatment with anticoagulants and restricted activity stabilized the condition, highlighting diagnostic and therapeutic considerations for this rare association.

Background:

  • A 37-year-old man with a history of herniated lumbar disc and allergic rhinosinusitis experienced VAD after sneezing.
  • Symptoms included C3 misalignment, headache, nystagmus, and vertigo.
  • MRI revealed cerebellar ischemia and right vertebral artery filling defect.

Findings:

  • CT Angiography confirmed right VAD with a thrombus, excluding aneurysmal signs.
  • Transcranial Doppler showed no increased blood flow in the affected artery.
  • Anticoagulant therapy (Apixaban) was initiated for 3 months.

Implications:

  • This case highlights the diagnostic challenges and therapeutic strategies for VAD with cerebellar infarction.
  • Management involved anticoagulation, work restrictions, and avoidance of strenuous physical activity.
  • Follow-up imaging indicated a stable condition, underscoring the importance of conservative management.
