    Background:

    • Developing effective physical activity interventions for moderate-to-severe multiple sclerosis (MS) is crucial.
    • Adherence to exercise programs remains a challenge in this population.

    Purpose of the Study:

    • To evaluate adherence to a Web-based, individualized exercise program for individuals with moderate-to-severe MS.
    • To explore secondary outcomes including quality of life, anxiety, depression, and functional mobility.

    Main Methods:

    • Randomized controlled trial with a 2:1 allocation ratio.
    • Intervention: Physiotherapist-guided Web-based home exercise program.
    • Comparator: Physiotherapist-prescribed written home exercise program.
    • Primary outcome: Exercise adherence over 26 weeks.

    Main Results:

    • No significant difference in adherence between the Web-based (38.9 ± 28.1 sessions) and written (34.6 ± 40.8 sessions) groups.
    • Nearly 50% of participants exercised at least twice weekly for 13 weeks.
    • Adherence was highest in the Web-based subgroup of wheelchair users.
    • Medium effect sizes observed for HADS anxiety and TUG in ambulatory participants.
    • No adverse events reported.

    Conclusions:

    • Web-based physiotherapy demonstrated comparable exercise adherence to traditional methods in moderate-to-severe MS.
    • The Web-based approach is safe, with no negative impact on secondary outcomes.
    • Further research is warranted to explore the specific benefits for wheelchair users in Web-based physiotherapy programs.
