Summary

New ferritin reference intervals (RI) were established for pediatric and adult populations, addressing limitations of older RIs. These updated RIs account for age and sex, improving diagnostic accuracy for ferritin testing.

Background:

  • Current ferritin reference intervals (RI) are outdated and may not reflect demographic and lifestyle changes.
  • Existing RIs, such as those for the Roche Elecsys® immunoassay, lack age-specific data for pediatric and geriatric groups.
  • Female RIs were primarily based on premenopausal volunteers, limiting their applicability.

Purpose of the Study:

  • To establish robust, age- and sex-specific reference intervals for ferritin.
  • To address the limitations of existing ferritin RIs in diverse populations.
  • To improve the accuracy of ferritin testing in clinical practice.

Main Methods:

  • Retrospective analysis of 5 years of ferritin measurements from 1438 unique patients (7 months to 91 years).
  • Exclusion of patients with diagnoses known to affect ferritin levels.
  • Development of continuous, age- and sex-dependent RIs, followed by division into clinically relevant age categories.

Main Results:

  • Established new reference intervals for males (pre-adolescent, adolescent, adult) and females (pediatric, adult, older adult).
  • Ferritin RIs were fitted as a continuous function of age for both sexes.
  • The new RIs were validated using specimens from healthy donors.
