Background:

  • Bronchoscopy is a common diagnostic procedure with a low risk of complications.

Purpose of the Study:

  • To identify and evaluate the risk factors associated with post-bronchoscopy pneumonia.
  • To determine if specific patient characteristics or procedural elements increase pneumonia risk.

Main Methods:

  • Retrospective case-control study.
  • Data collected from 2,265 patients undergoing 2,666 diagnostic bronchoscopies (April 2006 - November 2011).
  • Patients with post-bronchoscopy pneumonia (n=37) were matched with a control group for age and sex.

Main Results:

  • Post-bronchoscopy pneumonia occurred in 1.4% of patients.
  • Tracheobronchial stenosis was significantly more prevalent in the pneumonia group (75.7% vs 18.9%, p<0.01).
  • Primary lung cancer diagnosis was also higher in the pneumonia group (75.7% vs 43.2%, p<0.01).
  • Multivariate analysis identified tracheobronchial stenosis as an independent risk factor (OR: 7.8, 95% CI: 2.5-24.2).

Conclusions:

  • Tracheobronchial stenosis is a key independent risk factor for post-bronchoscopy pneumonia.
  • The findings emphasize the importance of considering airway anatomy when assessing pneumonia risk post-procedure.
  • Further research may explore targeted preventative strategies for high-risk patients.
