Background:

  • The development of effective vaccines against SARS-CoV-2 has been hampered by a lack of clearly defined correlates of immunity.
  • While neutralizing antibodies are associated with protection against other viruses, their sufficiency for SARS-CoV-2 protection in the general population is uncertain.

Purpose of the Study:

  • To investigate the early evolution of the humoral immune response in hospitalized patients with moderate to severe SARS-CoV-2 infection.
  • To identify distinct humoral immune trajectories associated with disease severity and outcomes.

Main Methods:

  • Analysis of humoral immune responses, including IgM, IgA, and IgG antibody levels.
  • Assessment of Fc gamma receptor binding and Fc effector function in 193 hospitalized individuals.
  • Comparison of immune responses between survivors and non-survivors, and between moderate and severe disease groups.

Main Results:

  • Patients with severe disease who did not survive exhibited attenuated IgG responses, compromised Fc gamma receptor binding, and reduced Fc effector activity.
  • Robust IgM and IgA responses were observed in both survivors and non-survivors with severe disease.
  • Individuals with moderate disease displayed delayed but ultimately mature humoral responses.

Conclusions:

  • Deficient humoral development, rather than disease-enhancing immunity, characterized non-survivors of severe SARS-CoV-2 infection.
  • Distinct humoral immune trajectories are associated with the resolution of SARS-CoV-2 infection.
  • Early development of functional humoral immunity is essential for effective SARS-CoV-2 infection control.
