Catastrophic Human Error in Assisted Reproductive Technologies: A Systematic Review

Human errors in assisted reproductive technologies (ART) labs can cause devastating patient outcomes. This review identifies 22 adverse events, including wrong sperm/embryo mix-ups and cryostorage failures, and proposes preventative strategies to improve ART safety.
