Summary

Facial flushing is not typically linked to hypoglycemia. This study details the first reported case of repeatable facial flushing occurring specifically during insulin-induced hypoglycemia in a diabetic patient.

Background:

  • Facial flushing is rarely associated with hypoglycemia.
  • Diabetics on chlorpropamide may experience flushing with alcohol (antabuse-like reaction).
  • Flushing directly secondary to hypoglycemia has not been previously documented.

Observation:

  • A case study of a diabetic patient experiencing significant erythematous facial flushing.
  • The flushing was consistently reproducible with each episode of insulin-induced hypoglycemia.
  • Endocrine workup excluded carcinoid syndrome and pheochromocytoma.

Findings:

  • This is the first reported instance of facial flushing occurring as a direct symptom of insulin-induced hypoglycemia.
  • The patient's flushing was unrelated to alcohol consumption.
  • The flushing resolved and did not recur when hypoglycemia was avoided.

Implications:

  • Facial flushing should be considered as a potential, albeit rare, symptom of hypoglycemia, particularly in diabetic patients.
  • This finding expands the known clinical manifestations of hypoglycemia.
  • Further research may be warranted to understand the mechanism linking hypoglycemia and facial flushing.
