Migraine attacks cause pupil size reduction and altered accommodation, suggesting subtle oculosympathetic hypofunction. Ocular higher-order aberrations (HOAs) remained unchanged during attacks.
Area of Science:
- Ophthalmology and Neuroscience
- Autonomic Nervous System Function
Background:
- Migraine is a complex neurological disorder with potential ocular manifestations.
- Understanding ocular changes during migraine attacks is crucial for diagnosis and management.
Purpose of the Study:
- To evaluate pupil size, accommodation, and ocular higher-order aberrations (HOAs) in migraine patients during attacks.
- To compare these ocular parameters between migraine attacks, interictal periods, and healthy controls.
Main Methods:
- Prospective case-control study involving 24 migraine patients and 24 healthy controls.
- Measurements included pupil size, accommodative responses (0-5D), and HOAs using a Hartmann Shack aberrometer.
- Patients were assessed during both interictal and ictal phases.
Main Results:
- Pupil size significantly decreased during migraine attacks (5.85 mm) compared to the interictal phase (6.05 mm).
- Accommodative response increased significantly during migraine attacks for stimuli between 1.5-5D.
- No significant changes in HOAs were observed; no parameters differed significantly from controls.
Conclusions:
- Findings suggest subtle oculosympathetic hypofunction during migraine attacks.
- Autonomic dysfunction during migraine appears to affect ocular accommodation.
- Ocular HOAs are not significantly altered during migraine attacks.
