Corynebacterium species can cause severe chronic urinary tract infections, known as encrusted cystitis (EC). Early diagnosis and treatment in pediatric patients, though rare, are crucial for a positive outcome.
Area of Science:
- Urology
- Infectious Diseases
- Microbiology
Background:
- Corynebacterium species are often misidentified as contaminants in urine cultures.
- Encrusted cystitis (EC), a severe chronic urinary tract infection, can lead to significant morbidity and mortality.
- EC is predominantly observed in elderly and immunocompromised individuals, with pediatric cases being exceedingly rare.
Observation:
- A female adolescent on high-dose corticosteroids presented with persistent dysuria following urinary catheterization.
- Bladder ultrasound revealed abnormalities, and cystoscopy showed characteristic signs consistent with EC.
- The patient's symptoms began 4 months prior to diagnosis and treatment initiation.
Findings:
- Diagnosis of encrusted cystitis (EC) was confirmed in a pediatric patient.
- The patient, an adolescent on high-dose steroids, experienced persistent dysuria post-catheterization.
- Prompt treatment led to an excellent clinical response, indicating the efficacy of intervention.
Implications:
- This case highlights the importance of considering Corynebacterium species and EC in the differential diagnosis of persistent urinary symptoms in pediatric patients, even in rare instances.
- Early recognition and appropriate management of EC in adolescents can lead to favorable outcomes.
- Further research into the diagnosis and treatment of EC in pediatric populations is warranted.
