Decrease in Mucosal IL17A, IFNγ and IL10 Expressions in Active Crohn's Disease Patients Treated with High-Dose

High-dose vitamin D, with or without infliximab, reduced inflammatory cytokine expression in Crohn's disease (CD) patients. However, vitamin D monotherapy did not significantly improve disease activity.

- Administration of infliximab (5 mg/kg) or placebo at weeks 0, 2, and 6; oral vitamin D (200,000 IU bolus then 20,000 IU/day) or placebo for 7 weeks.
- Endoscopic assessment, mucosal mRNA cytokine expression analysis (IL17A, IFNγ, IL10), C-reactive protein (CRP), fecal calprotectin, and Harvey-Bradshaw Index (HBI) measurements.
Main Results:
- High-dose vitamin D, as monotherapy or combined with infliximab, decreased IL17A, IFNγ, and IL10 expression.
- Vitamin D monotherapy did not significantly reduce disease activity, CRP, or fecal calprotectin levels.
- Combination therapy with infliximab and vitamin D was not significantly superior to infliximab monotherapy in clinical outcomes.
Conclusions:
- High-dose vitamin D therapy, alone or with infliximab, reduces specific mucosal cytokine expressions (IL17A, IFNγ, IL10) in Crohn's disease.
- This modulation of cytokine expression did not translate to a statistically significant decrease in overall disease activity.
- Further research may be needed to clarify the clinical role of vitamin D in managing Crohn's disease, especially in combination with biologics.
