The pineal gland, a diminutive endocrine structure named for its pinecone-shaped appearance, is situated atop the third ventricle within the diencephalon region of the forebrain. Summary

Pineal cysts commonly found on MRI scans tend to remain stable in volume over time. These cysts do not reliably correlate with symptoms and usually do not require long-term neurosurgical follow-up.

Background:

  • Pineal cysts are frequently identified as incidental findings on brain MRI.
  • A clear management strategy for pineal cysts lacking hydrocephalus or other specific syndromes is currently unavailable.

Purpose of the Study:

  • To identify factors predicting pineal cyst volume changes.
  • To determine factors associated with the need for surgical intervention in patients with pineal cysts.

Main Methods:

  • Retrospective chart review of 98 patients diagnosed with pineal cysts between 2005 and 2018.
  • Analysis of initial and follow-up MRI scans to measure cyst volume (cm³).
  • Review of patient symptoms, demographics, and management decisions.

Main Results:

  • Fifty-five patients met the study criteria, with follow-up ranging from 4 months to 10 years.
  • Observed pineal cysts showed minimal mean volume change (0.051 cm³), with a median change of 0 cm³.
  • No significant association was found between patient symptoms, referral source, age, and cyst volume changes or surgical intervention.

Conclusions:

  • Pineal cyst volumes generally remain stable on serial MRI.
  • Cyst volume does not reliably correlate with patient symptoms.
  • Pineal cysts typically do not necessitate long-term neurosurgical follow-up unless specific complications arise.
