Late onset multiple sclerosis is associated with more severe ventricle expansion

Late-onset multiple sclerosis (LOMS) patients show greater brain atrophy and ventricle expansion compared to adult-onset MS patients. This indicates a potentially more aggressive disease course in LOMS.

Purpose of the Study:
- To compare MRI-derived brain atrophy rates in persons with LOMS (PwLOMS) against age- and disease duration-matched persons with adult-onset MS (PwAOMS).

- 870 individuals with MS (290 PwLOMS, 290 age-matched PwAOMS, 290 disease duration-matched PwAOMS) and 150 healthy controls (HCs) were studied over 3-5 years.
- T2-lesion volume (LV), lateral ventricular volume (LVV), and whole brain volume (WBV) were measured using MRI.
- Expanded Disability Status Scale (EDSS) and Multiple Sclerosis Severity Score (MSSS) were assessed.
Main Results:
- PwLOMS exhibited significantly higher baseline and follow-up MSSS compared to both PwAOMS groups (p<0.026).
- PwLOMS showed significantly greater percent LVV change (14.3% vs. 9.3%, p=0.001) and annualized LVV change (4.1% vs. 1.6%, p<0.001) versus age-matched PwAOMS.
- All persons with MS had greater WBV loss and LVV expansion than HCs.
Conclusions:
- Persons with LOMS experience greater ventricle expansion compared to persons with adult-onset MS.
- Higher MSSS in PwLOMS suggests a more severe disease course.
