[A case report of wandering spleen]

Observation:

• A 43-year-old woman presented with acute abdominal pain and splenomegaly.
• Physical examination revealed splenic migration, and CT confirmed a wandering spleen with vascular pedicle torsion.

Findings:

• Computed tomography is essential for diagnosing wandering spleen and associated complications like torsion.
• Surgical treatment, including splenopexy or splenectomy, is indicated for symptomatic wandering spleen.

Implications:

• Wandering spleen can lead to severe complications including splenic infarction and portal hypertension.
• This rare diagnosis should be considered in cases of recurrent abdominal pain, painful splenomegaly, or pelvic masses.
