Summary
Multiple schwannomas with a pseudoglandular element, a rare benign nerve sheath tumor, can occur under the tongue. Early recognition is crucial for accurate diagnosis and appropriate treatment planning.
Area of Science:
- Oncology
- Pathology
- Surgical Oncology
Background:
- Schwannomas are rare, benign nerve sheath tumors, typically solitary and encapsulated.
- While common variants exist, multiple schwannomas with a pseudoglandular element are exceptionally rare, especially when occurring synchronously under the tongue.
Observation:
- A 53-year-old male presented with multiple, growing, asymptomatic masses under the tongue.
- Imaging revealed three cystic-solid, low-density neoplasms. Preoperative diagnosis was mucoepidermoid carcinoma.
- The patient had no history of neurofibromatosis 2 or related conditions.
Findings:
- Histopathology confirmed multiple schwannomas with a pseudoglandular element.
- Surgical excision via an intraoral approach was performed.
- No recurrence or new tumors were observed at 15-month follow-up.
Implications:
- This case highlights the rare occurrence of multiple schwannomas with pseudoglandular elements presenting as sublingual masses.
- Oral surgeons must consider this diagnosis for sublingual masses, differentiating it from mucoepidermoid carcinoma due to distinct prognoses.
