Plasma exchange in ANCA-associated vasculitis: the pro position

Background:

• Plasma exchange (PLEX) removes circulating antibodies and was historically reserved for severe anti-neutrophil cytoplasmic antibody-associated vasculitis (ANCA- vasculitis) presentations.
• The PEXIVAS trial investigated PLEX in a broader ANCA-vasculitis patient population with impaired kidney function.

Purpose of the Study:

• To evaluate the efficacy of routine plasma exchange in addition to glucocorticoids for severe ANCA-associated vasculitis.
• To determine if PLEX offers benefits beyond standard treatment in patients with reduced glomerular filtration rates.

Main Methods:

• The Plasma Exchange and Glucocorticoids in Severe ANCA-Associated Vasculitis (PEXIVAS) trial randomized patients with ANCA-vasculitis and glomerular filtration rate <50 mL/min/1.73 m².
• Patients received either standard care plus PLEX or standard care alone, with outcomes assessed over nearly 3 years.

Main Results:

• Routine PLEX administration did not reduce the composite endpoint of end-stage kidney disease or death in severe ANCA-vasculitis patients.
• A subset of patients with alveolar hemorrhage showed a trend towards better outcomes with PLEX, aligning with observational data.

Conclusions:

• Routine plasma exchange offers no significant additional benefit for the composite outcome of end-stage kidney disease or death in severe ANCA-vasculitis.
• Further research is needed to clarify the role of PLEX in ANCA-vasculitis, particularly for patients with alveolar hemorrhage, before its complete discontinuation.
