Benign Metastasizing Leiomyoma (BML) is a rare condition where uterine leiomyomas spread to the heart. This case highlights cardiac BML, emphasizing its metastatic potential despite benign histology.

Area of Science:

  • Cardiology
  • Gynecologic Oncology
  • Pathology

Background:

  • Benign Metastasizing Leiomyoma (BML) is a rare condition characterized by the spread of uterine leiomyomas.
  • Cardiac involvement in BML is exceptionally rare and often asymptomatic.
  • BML primarily affects premenopausal women with a history of uterine leiomyomas.

Observation:

  • A 42-year-old woman presented with three tumors in the right side of her heart.
  • These cardiac masses were discovered two years after a hysterectomy for leiomyomatosis.
  • The patient developed cardiac failure secondary to the intracardiac tumors.

Findings:

  • Surgical resection of the cardiac tumors confirmed the diagnosis of uterine leiomyoma.
  • A co-existing ovarian mass was also identified and removed.

  • Histological examination revealed that although benign, the BML exhibited metastatic characteristics.
  • Implications:

    • This case underscores the importance of considering BML in the differential diagnosis of intracardiac masses in women.
    • Early recognition and surgical intervention are crucial for managing cardiac BML.
    • BML, despite its benign histological nature, possesses metastatic potential requiring vigilant monitoring.
