Summary

Pneumonitis distal to airway tumors is often assumed to be infectious. However, this study found noninfectious consolidation is the usual cause of radiographic changes in lung cancer patients.

Background:

Purpose of the Study:

  • To reevaluate the concept that infection causes pneumonitis distal to airway tumors.
  • To clarify the underlying pathologic findings responsible for radiographic changes in such cases.

  • A prospective study involving 50 consecutive patients undergoing pulmonary carcinoma resection.
  • Histopathologic examination of lung specimens to identify infectious and noninfectious changes.

Main Results:

  • Noninfectious histologic changes were identified in 42 out of 50 cases.
  • Infection-related findings were present in only nine specimens, primarily affecting airways and superimposed on noninfectious pneumonitis.

Conclusions:

  • Radiographic changes associated with airway obstruction in resected lungs are typically due to noninfectious parenchymal consolidation.
  • When infection is present, it predominantly affects airways and is unlikely to be radiographically apparent.
