Perceived invalidation from family and spouses can help identify fibromyalgia (FM) in chronic pain patients. This social pain is a key factor in distinguishing FM from other chronic pain conditions.
Area of Science:
- Rheumatology
- Pain Medicine
- Social Psychology
Background:
- Fibromyalgia (FM) is a complex condition often accompanied by polysymptomatology.
- The role of social pain and perceived invalidation in FM diagnosis is under-recognized.
- Understanding social pain's impact is crucial for a comprehensive approach to FM.
Purpose of the Study:
- To investigate the effectiveness of perceived invalidation in discriminating between fibromyalgia (FM) and non-FM chronic pain patients.
- To identify specific sources of invalidation that are most predictive of FM.
- To explore the diagnostic utility of social pain in the context of fibromyalgia.
Main Methods:
- 207 chronic pain patients were assessed for FM using rheumatologic evaluation.
- The Illness Invalidation Inventory (3*I) measured perceived invalidation from various sources.
- Receiver operator characteristic (ROC) and logistic regression analyses were employed to assess discriminatory power.
Main Results:
- Patients with FM reported higher levels of discounting and lack of understanding from spouses and family compared to non-FM patients.
- Invalidation from spouse and family sources significantly discriminated between FM and non-FM groups.
- Family discounting (OR 2.30) and spousal lack of understanding (OR 1.72) were significant predictors of FM.
Conclusions:
- Perceived invalidation, particularly from family and spouses, holds significant discriminatory power for identifying fibromyalgia.
- These findings support the inclusion of invalidation as a key component in the understanding and diagnosis of FM.
- Acknowledging social pain in FM diagnosis may shift the clinical paradigm for this condition.
