Background:

  • Thyrotoxic periodic paralysis (TPP) is an endocrine-neuromuscular disorder characterized by episodic hypokalemic muscle paralysis.
  • TPP predominantly affects individuals of Asian descent, with rare reports in other ethnicities, including Hispanic populations.

Observation:

  • A 28-year-old Mexican male presented with acute bilateral lower extremity weakness following a high-carbohydrate meal.
  • Clinical presentation was associated with severe hypokalemia (serum potassium 2.1 mmol/L) and concurrent hyperthyroidism.

Findings:

  • Intravenous potassium chloride administration rapidly corrected hypokalemia and resolved the muscle weakness.
  • Propranolol treatment and subsequent radioiodine therapy (iodine 131) effectively prevented recurrent paralytic episodes.

Implications:

  • This case highlights that thyrotoxic periodic paralysis, though infrequent, should be considered in the differential diagnosis of acute weakness in Hispanic individuals.
  • Early recognition and management of TPP are crucial for preventing morbidity and improving patient outcomes.
