Background:

  • Severe crushing closed-head trauma can lead to delayed neurological complications.
  • The mesencephalon (midbrain) is a critical brain region for various neurological functions.

Observation:

  • A 28-year-old man experienced abrupt onset of right mesencephalon deficits 7 months post-head trauma.
  • Computed tomography revealed a cystic midbrain lesion potentially communicating with the aqueduct.

Findings:

  • Clinical deterioration correlated with a cystic midbrain lesion.
  • Brain-stem auditory evoked potentials showed reduced wave V amplitude during contralateral ear stimulation post-deficit onset.

Implications:

  • Cystic midbrain lesions can cause delayed neurological deficits after head trauma.
