Dormia basket impacted during ERCP, resolved by laparoscopic bile duct approach: Case report

Background:

• Endoscopic retrograde cholangiopancreatography (ERCP) is a primary treatment for common bile duct (CBD) stones.
• Dormia basket impaction is an uncommon but challenging complication during ERCP stone removal.

Observation:

• Two cases of CBD stones treated with ERCP experienced Dormia basket impaction.
• Both cases were successfully managed using a laparoscopic approach to the bile duct.

Findings:

• Laparoscopic common bile duct exploration (LCBDE) effectively resolves CBD stones and associated ERCP complications.
• LCBDE, via transcystic or choledochotomy approaches, demonstrates high success rates (>95%) dependent on surgical expertise.

Implications:

• LCBDE offers a safe, effective, and minimally invasive alternative for complex choledocholithiasis and ERCP-related adverse events.
• This technique provides a valuable therapeutic option when other treatments fail, ensuring successful stone clearance and complication resolution.
