Background:

Intersigmoid hernia is an exceptionally rare internal hernia, with limited documented cases of laparoscopic repair.
This condition often presents with non-specific symptoms, making preoperative diagnosis challenging.

Observation:

A 50-year-old male presented with abdominal pain and vomiting, diagnosed preoperatively via CT scan as an incarcerated internal hernia.
Laparoscopic surgery revealed ileal herniation into the intersigmoid fossa, confirming an intersigmoid hernia.

Findings:

The incarcerated small bowel was successfully reduced laparoscopically without evidence of ischemia.
The hernial defect was widened, and the patient recovered well, despite a transient postoperative ileus.
