Summary

Mesopic microperimetry better reveals the structure-function link in eyes with drusen compared to scotopic tests. This relationship is weak overall but strengthens when analyzing specific locations with retinal pigment epithelium elevation.

Background:

  • Drusen are hallmark features of age-related macular degeneration (AMD).
  • Understanding the relationship between structural changes and visual function is crucial for monitoring AMD progression.

Purpose of the Study:

  • To investigate the structure-function relationship in eyes with drusen using mesopic and scotopic microperimetry.
  • To assess how structural parameters correlate with visual sensitivity loss under different lighting conditions.

Main Methods:

  • Analyzed structural data (green autofluorescence, OCT scans for RPE elevation and PRR) and functional data (mesopic/scotopic microperimetry) from 43 eyes with drusen.
  • Calculated normative values from 56 healthy eyes.
  • Used linear mixed models to measure the pointwise structure-function relationship, with sensitivity loss as the response variable.

Main Results:

  • Both mesopic and scotopic tests showed significant correlations between structural predictors and sensitivity loss.
  • Mesopic microperimetry demonstrated a better, though weak, structure-function relationship (best R² = 0.09).

Conclusions:

  • Mesopic microperimetry offers a superior structure-function correlation compared to scotopic conditions in eyes with drusen.
  • The weak overall relationship may stem from early functional damage and limited tested areas.
  • Isolating drusen locations, particularly with the cyan test, enhances this correlation.
