Background:

  • Thyroidectomy volume is linked to fewer complications, but its effect on incidental parathyroidectomy is less understood.
  • Central neck dissection (CND) poses a higher risk to parathyroid glands due to their location.

Purpose of the Study:

  • To investigate the association between surgical volume and incidental parathyroidectomy during thyroidectomy with or without CND.
  • To identify factors associated with incidental parathyroidectomy and its impact on parathyroid function.

Main Methods:

  • Retrospective analysis of 1,114 thyroidectomies and 396 CNDs performed by 7 surgeons.
  • Univariate and multivariable logistic regression analyses were used to evaluate incidental parathyroidectomy, hypoparathyroidism, and hypocalcemia.

Main Results:

  • Incidental parathyroidectomy occurred in 22.4% of surgeries.
  • Lower incidental parathyroidectomy rates were associated with higher thyroidectomy (R²=0.77) and CND (R²=0.93) volumes.
  • Low-volume surgeons, extrathyroidal extension, and prophylactic/therapeutic CND were significant factors for incidental parathyroidectomy. Incidental parathyroidectomy increased the risk of temporary and permanent hypoparathyroidism.

Conclusions:

  • Higher surgical volume is associated with a lower rate of incidental parathyroidectomy.
  • Increased lymph node yield in CND did not correlate with higher parathyroid-related morbidity.
  • Surgical volume is a key factor in optimizing oncologic resection and minimizing complications in thyroid surgery.
