Acute suppurative thyroiditis is rare, especially Salmonella thyroid abscesses. Two immunocompromised patients developed Salmonella thyroid abscesses without gastrointestinal issues, requiring surgical intervention for recovery.

Area of Science:

  • Endocrinology
  • Infectious Diseases
  • Surgical Pathology

Background:

  • Acute suppurative thyroiditis is a rare condition (<1% of thyroid diseases) due to the thyroid gland's protective anatomical and physiological features.
  • Commonly caused by Staphylococci and Streptococci, rarer pathogens like Salmonella spp. are infrequently reported, with only 28 cases of Salmonella thyroid abscess documented globally by 2020.

Observation:

  • This report details two cases of Salmonella enteritidis thyroid abscess in immunocompromised patients presenting with neck swelling, dysphagia, and sepsis.
  • Neither patient had gastrointestinal involvement, a typical association with Salmonella infections.
  • Both cases necessitated surgical intervention due to disease severity and failure of medical management.

Findings:

  • Successful recovery was achieved in both patients following surgical drainage and targeted antibiotic therapy based on pathogen culture and sensitivity.

  • The cases suggest a potential re-emergence of Salmonella thyroid abscess, particularly in immunocompromised individuals.
  • Implications:

    • Highlights the importance of a high index of suspicion for Salmonella thyroid abscess in immunocompromised patients, even without gastrointestinal symptoms.
    • Emphasizes prompt diagnosis and appropriate management, including antibiotics, percutaneous aspiration, or surgical drainage, for effective treatment of this rare condition.
