Background:

  • Optical coherence tomography angiography (OCTA) is an emerging non-invasive imaging technique.
  • OCTA enables quantitative assessment of retinal microvasculature, including vessel density (VD).

Purpose of the Study:

  • To systematically review longitudinal studies on peripapillary and macular vessel density (VD) changes in glaucoma patients using OCTA.
  • To synthesize current evidence on the rate and pattern of VD changes in glaucoma.

Main Methods:

  • Systematic literature search of MEDLINE, Scopus, ISI Web of Science, and Google Scholar up to September 20, 2019.
  • Inclusion criteria: prospective studies, quantitative assessment of longitudinal VD changes in glaucoma, and a minimum follow-up of 3 months.
  • Data extraction focused on reported rates of VD change and outcomes after glaucoma surgery.

Main Results:

  • Ten studies involving 4516 participants were included.
  • Reported annual rates of VD change in glaucoma ranged from 0.036/year to 1.08/year and 1.3% to 3.2%.
  • Significant differences in VD change rates were observed between glaucoma patients and healthy controls. Post-surgical VD changes were variable.

Conclusions:

  • OCTA is a promising non-invasive tool for glaucoma research and monitoring.
  • Significant heterogeneity in methodologies and measurements across studies precluded meta-analysis.
  • Standardization of OCTA protocols and VD measurements is crucial for future longitudinal glaucoma studies and meta-analyses.
