Summary

Xanthelasma palpebrarum can present with severe eyelid disfigurement even without high cholesterol. This case highlights the importance of considering xanthelasma beyond lipid abnormalities.

Area of Science:

  • Dermatology
  • Ophthalmology

Background:

  • Xanthelasma palpebrarum (XP) is a common xanthoma variant characterized by yellowish deposits on the eyelids.
  • XP is typically associated with dyslipidemia, particularly elevated low-density lipoprotein (LDL) cholesterol.

Observation:

  • This report details a rare case of XP with extensive and disfiguring involvement of all four eyelids.
  • The patient presented with significant cosmetic and functional impairment due to the lesions.

Findings:

  • Despite the extensive presentation, the patient exhibited normal serum lipid profiles, including total cholesterol, LDL, and triglycerides.
  • This finding challenges the conventional association between XP and hyperlipidemia.

Implications:

  • This case underscores that XP can occur in normolipidemic individuals, suggesting other underlying pathogenetic mechanisms.
  • Further research is warranted to explore non-lipid-related factors contributing to XP development.
  • Clinicians should consider XP diagnosis irrespective of serum lipid levels, especially in cases of significant eyelid involvement.
